What this route actually is
Medicine in Pakistan is a long, regulated route with a single front door: MDCAT. Everything after it — which college you attend, whether you pay subsidised or private fees, how early you can specialise — follows from one aggregate calculated on Matric, FSc and that test.
It suits students who can sustain effort over a decade rather than a semester. The degree itself is five years, but the point at which you are earning independently as a specialist is considerably later, and the years in between are structured by institutions rather than chosen by you.
What the work involves
- House job and early postgraduate years are shift-based hospital work, including nights and weekends, in wards that are usually understaffed.
- Clinical practice is as much communication as diagnosis — explaining to families, managing expectations, and documenting carefully.
- Specialists split time between outpatient clinics, procedures or theatre, and teaching juniors.
Worth knowing before you commit
- The bottleneck is public seats, not ability. Far more students clear MDCAT than there are subsidised places, and private medical college fees are a different financial proposition entirely.
- BDS is a separate degree with its own career, not a consolation MBBS. In Punjab it has historically closed only slightly below MBBS, so it is not a low-merit fallback either.
- Specialisation, not the MBBS itself, determines most of what your working life looks like. That choice is made years after admission, under its own competition.
The route, in order
- 01
FSc Pre-Medical or equivalent2 years
Biology is required. A-Levels and other equivalents are accepted through IBCC equivalence.
- 02
MDCAT
The national admission test. You must clear the qualifying score for the cycle before your aggregate is considered at all.
- 03
MBBS5 years
Preclinical years followed by clinical rotations across the major specialties.
- 04
House job1 year
Supervised rotations in a teaching hospital. Required before independent practice.
- 05
Postgraduate training
FCPS through the College of Physicians and Surgeons Pakistan, or an MD/MS through a university. Several further years, and competitive in the popular specialties.
Who regulates this
Pakistan Medical and Dental Council. Registration is required to practise. Confirm current requirements and any examination directly with the Council — they have changed more than once in recent years.
If you don't get in
Most students who aim at a competitive route do not get a place on the first attempt. These are the routes people actually take next.
- Doctor of Physical Therapy, Pharm-D, BS Nursing and the allied health degrees are clinical careers in their own right, with their own regulators and shorter routes to practising.
- BS Biotechnology, Biochemistry and Microbiology lead to laboratory, research and industry work for students who want the science without the clinic.
- Repeating MDCAT is common and legitimate, but treat it as a decision with a cost — a year — rather than a default.
